I do hereby declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity or other illness that would prevent my participation or use of equipment in this program. I do hereby further assume all responsibly for my participation, activities and use of equipment in my activities. Recorded programs will be edited to remove images of class participants. However, there is still a possibility that participants may be visible during a class session. As such, I hereby give my consent to authorize Country Fitness use my video image in a class recording. I also understand that I can control my own video display and keep it turned off during a live class.

Cancellation Policy

The course cancellation policy includes a full refund less a $10 administrative fee for any cancellations before the first class. No other refunds available; however, registrations are transferable to another attendee.

Missed classes may not be made up; however, class recordings are available.
