    I do hereby declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity or other illness that would prevent my participation or use of equipment in this program.  I do hereby further assume all responsibly for my participation, activities and use of equipment in my activities.  By checking the box below, I assume all risk for my health and well being and hold harmless of any responsibility of the instructor or any persons involved with this program.
Cancellation Policy

The course cancellation policy includes a full refund less a $10 administrative fee for any cancellations before the first class.  No other refunds available; however, registrations are transferable to another attendee.  

In the case of inclement weather, classes will be cancelled.  Visit this website for weather-related class cancellations.

Missed classes may not be made up.
